Plan participation is subject to change. Please verify plan participation at each visit.

You are responsible for knowing your plan benefits and requirements. If your plan requires a PCP, our facility or physicians must be designated as your PCP for us to file your claim.

PNBM requires payment at the time of service. We accept Visa, MasterCard, American Express, and Discover cards. Patients with standard co-pay are encouraged to make their payments at the time of check-in. Other insurance, private pay, or third party liability accounts are payable at the time of service.
